MY SERVICES

Advocacy

Advocacy in residential, long term care, and medical settings
Making arrangements for and monitoring in-home care services
Assistance with relocation from one living situation to another
Evaluation of eligibility for services and assistance
Coordination of medical care with all doctors and caregivers
Assistance with placement at a long term care facility

Counselor Services

Identifying instances when medical care and other services are needed and making appropriate referrals to procure such services
Transportation for non-emergency services such as doctor appointments and grocery shopping
Assistance with addressing driving issues
Services as liaison between family members, client, and other service providers
Preparation of documents appoint financial and health care representatives

Financial Services

Help with financial matters, such as Medicare, Medicaid and medical insurance and referral to elder law attorneys as needed

Bill paying assistance

Service as conservator or attorney in fact (acting pursuant to a power of attorney)

Helper/Companion Services

Your Day: Up to 8 hours and 100 miles $250: shopping trips, museums, movies, restaurants, theater, visit a friend, walk a boardwalk or a trail

Concierge services; good morning calls, errands, pet services

Companionship, empowerment, and support in living independently

If you have a need that is not addressed here, please ask. It may be something I have not anticipated but can either provide or arrange for.

MY FEES

Hourly fees range from $20 to $80. Flat fee services may be negotiated. Fees are established as part of the service contract.
